Patient Registration Packet
Save time and complete the registration packet prior to your appointment. Please bring a copy of your insurance card, photo ID, and the completed packet to your appointment.
If scheduling a minor, please complete the Minor Consent Form
Online Bill Pay
We provide this service to you at no charge so you can access and pay your medical bills online conveniently and securely. You must be using Chrome or Firefox to pay from the links to the right. Policies
We work hard to keep waiting times minimal and your experience in our offices enjoyable. Please keep in mind that we have a 15-minute grace period and arriving late can result in being rescheduled. In the event that you need to cancel, please call the office at least 24 hours in advance for any medical appointment and at least 48 hours in advance for any surgical/cosmetic appointment.
Failure to give 24 hours’ notice of cancellation for a medical appointment may result in a charge of $50; failure to give 48 hours’ notice of cancellation for a surgical/cosmetic appointment may result in a charge of $150.
